What To Expect
Please arrive fifteen minutes prior to your first appointment to complete new client paperwork and bring insurance information for John Lawson and Eve Geisey. For all other providers your new patient paperwork and insurance information will be on the Simple Practice app and sent to you upon scheduling an appointment.
Payment Options
Insurances accepted and private pay rates are listed under each provider’s name in the Meet Us Section
All Copays, Deductible amounts, and Private pay amounts are due at the time of service. We currently do not have any providers who accept Medicare at this time. If there is a question about how much you will owe please call the office and we will be happy to look up your insurance.
